Vanguard Student-Athletes,

Below are the necessary forms that need to be completed and returned to Vanguard Athletics, as well as some additional information. If you have any questions, please contact Associate Athletic Director Nolan Steputis at 714-556-3610, ext. 2302. Thank you.


Physical Exam Procedures: (Read over this for better understanding of the physical procedure)


Returning Student Athlete Physical Packet: (Print out, complete, and mail to coach before August 1)



New Student Athlete Physical Packet: (If this is going to be your first time participating in Vanguard Athletics, print this form out, complete it, and mail to coach before Aug. 1.  There is a $50.00 physical fee for all new student athletes.  Please make check payable to Vanguard University and either mail with packet or bring the payment the day of physicals)



Letter concerning sickle cell: (Stating Vanguard's adopted ruling of the sickle cell trait status)



Sickle cell Authorization of Release: (Print this form out and take it to your doctor to complete, and then mail to your coach)



Information about sickle cell: (helpful information about the sickle cell trait)



Try-out waiver: (If you are trying-out for a team while on a visit, please sign this form and bring it to campus with you)



Transfer form: (If you are transferring to Vanguard, please complete this form and return it to the Athletic Department as soon as possible---This will help with your eligibility process.)



Input your Athlete Information for the Athletic Administration (must know password and you will receive password once on campus)
